How Gabapentin + Nortriptyline works

  • Gabapentin binds to specific sites on voltage-gated calcium channels, inhibiting the release of neurotransmitters. Neurotransmitters are chemical messengers that are responsible for carrying pain signals from one nerve cell to the next target cell. By reducing the release of neurotransmitters, gabapentin helps relieve nerve pain. It also prevents abnormal electrical activity in damaged nerves.
  • Nortriptyline, on the other hand, increases the levels of serotonin and noradrenaline in the brain. It, thus, helps regulate mood and prevents pain signals from being transmitted.

  • This combination is recommended for adults and is not intended to be used for children.

Contraindications 

  • This combination is contraindicated in patients with a history of hypersensitivity to gabapentin or nortriptyline and in patients who are on monoamine oxidase inhibitors as its concurrent use can cause serotonin syndrome in which a combination of symptoms occur such as agitation, confusion, tremor, myoclonus and hyperthermia. This combination is also contraindicated in patients with recent myocardial infarction, any degree of heart block or disorders of cardiac rhythm and coronary artery insufficiency.

Other Medications To Avoid With This Medicine

  • Avoid taking sympathomimetic agents (adrenaline), antihypertensives (clonidine, reserpine), tricyclic antidepressants (amitriptyline), antiarrhythmics (quinidine), antihistamines (terfenadine), antipsychotics (sertindole), opiate analgesics (methyldopa), diuretics (furosemide), antifungals (fluconazole). These medications may alter the safety and effectiveness of this combination. Precautions should be taken while using this combination with antacids containing aluminium & magnesium, cytochrome P450 inducers and valproic acid.

Overdose And Missed Dose

  • If an overdose occurs and leads to severe side effects or worsening of any of the symptoms, it needs to be managed as an emergency in a hospital setting. Therefore, strict adherence to the prescribed dose is essential.
  • If you miss a dose of this combination, take it as soon as you remember. But, if the next dose is near, skip the missed dose and resume the regular dosing regime.

Management Of Side Effects

  • Take this combination with food to reduce gastrointestinal side effects and continue taking it as prescribed. If side effects worsen or persist, consult your doctor for a possible treatment plan adjustment.

Use In Driving And Operating Machinery

  • Refrain from driving or operating machinery after taking this combination, as it can make you feel dizzy.

Use In Pregnancy And Lactation

  • This combination is not recommended during pregnancy and breastfeeding.

  • Caution is advised in older adults and in patients with preexisting liver and kidney issues.
