  • Esomeprazole + Levosulpiride is used to gastroesophageal reflux disease (GERD) in individuals who do not respond well to a single proton-pump inhibitor (PPI).
  • GERD occurs when stomach acid frequently flows back into the esophagus, causing symptoms like acidity, pain, and heartburn. This combination helps alleviate these symptoms by reducing acid production and improving gastrointestinal motility.

How Esomeprazole + Levosulpiride works

  • Esomeprazole works as a proton pump inhibitor (PPI), exerting its action on gastric acid (hydrochloric acid - HCl) secretion through specific inhibition of the H+/K+-ATPase enzyme system at the secretory exterior of the gastric parietal cell. As a weak base, esomeprazole undergoes concentration and conversion to its active form (i.e., the achiral sulphenamide) within the highly acidic environment of the secretory canaliculi of the parietal cell. In this locale, it inhibits the enzyme H+K+-ATPase (the acid/proton pump), effectively suppressing both basal and stimulated acid secretion. Through its targeted action on the proton pump, esomeprazole disrupts the final stage of acid production, reducing gastric acidity.
  • Levosulpiride primarily functions as a dopamine D2 antagonist, targeting the muscular D2 receptors activated by dopamine to induce a direct relaxant effect on the gastrointestinal tract. It enhances gut motility by inhibiting D2 receptors in the lower oesophagal sphincter (LES) and stomach. Additionally, levosulpiride serves as an agonist on serotonin 5-HT4 receptors, elevating acetylcholine levels and thereby promoting increased gastrointestinal motility. Its selective antagonist activity on D2 receptors in the central nervous system, particularly in the postrema area of the 4th ventricle, contributes to its antiemetic effects. Acting as a weak inhibitor of 5-HT3 receptors and a moderate agonist at the serotonergic (5-HT4) receptor, levosulpiride exhibits therapeutic efficacy in gastrointestinal disorders, notably reducing nausea and vomiting.

Prior consultation with a doctor is essential before considering the use of any prescription or over-the-counter medications to ensure the safety and efficacy of the combined treatment plan.

Overdose and Missed Dose

  • Overdose of this combination can cause confusion, drowsiness, blurred vision, tachycardia, nausea, diaphoresis, flushing, headache, and dry mouth. Inform your doctor regarding these effects and follow the advice accordingly.
  • If you forget to take a dose of this combination, take it as soon as you remember. However, avoid repeating the dose.

  • This combination may affect your ability to drive or operate heavy machinery. Avoid driving if you feel dizzy after taking this combination.

Use in Pregnancy and Lactation

  • Esomeprazole + Levosulpiride is not recommended during pregnancy and while breastfeeding.
