Uses of Eplerenone

  • The main use of Eplerenone is to manage high blood pressure and heart failure, especially in patients who have suffered a recent heart attack. It belongs to the group of medicines known as mineralocorticoid receptor antagonists (MRAs).

  • Additionally, it is used to improve survival in stable patients with left ventricular dysfunction (ejection fraction ≤40%) and evidence of congestive heart failure after a recent heart attack.

  • High blood potassium levels (Hyperkalemia)

  • Dizziness

  • Headache

  • Digestive upset, including diarrhoea and nausea

  • Fatigue and flu-like symptoms

How Eplerenone works

  • Eplerenone is a mineralocorticoid receptor antagonist.

  • It works by blocking the action of a hormone called aldosterone, which usually acts on the kidneys to promote absorption of salt and water while causing the kidney to get rid of potassium. By blocking aldosterone, Eplerenone allows the kidneys to excrete more salt and water and retain more potassium.

  • Eplerenone can be taken orally (tablet).

Age and Dose Restrictions

  • This medicine is intended for adults only and is not recommended for children.

Contraindications

  • Eplerenone is contraindicated in patients with known allergies to this medication.

  • Additionally, it is contraindicated in patients with hyperkalemia, reduced creatinine clearance, and in those using strong CYP3A4 inhibitors. It is also contraindicated in patients with type 2 diabetes with microalbuminuria and those with high serum creatinine levels.

Other Medications to Avoid

  • Eplerenone may interact with strong CYP3A4 inhibitors (such as ketoconazole, and itraconazole), ACE inhibitors (like lisinopril, ramipril), ARBs (like losartan, valsartan), NSAIDs (like ibuprofen, naproxen), and moderate CYP3A inhibitors (such as erythromycin, diltiazem).

Overdose and Missed Dose

  • An overdose of Eplerenone may lead to symptoms such as drowsiness, confusion, diarrhoea, rash, or increased heart rate. If you observe these symptoms, inform your doctor immediately.

  • In case you miss a dose of Eplerenone, take it as soon as possible. However, if it's close to the time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take extra medicine to make up for the missed dose.

Management of Side Effects

Most side effects are temporary, generally harmless and resolve when discontinuing this medicine. However, if you experience any severe side effects or worsening of any of the symptoms, please consult your doctor.

  • Drinking plenty of water may help manage some side effects, such as dizziness and headaches.

  • It is also suggested to take the medication after meals to avoid gastrointestinal issues like nausea and abdominal pain.

  • Resting and avoiding sudden movements can help manage fatigue and dizziness.

Use in Driving and Operating Machinery

  • Eplerenone may cause dizziness or fatigue. If this occurs, do not drive or operate heavy machinery until you feel well again.

Use in Pregnancy and Lactation

  • Eplerenone is not recommended for use during pregnancy or while breastfeeding, unless explicitly advised by a doctor. Always consult your doctor before taking this medication if you are pregnant or breastfeeding.

Caution in Other Conditions

  • Individuals with impaired kidney function or diabetes should exercise caution when taking Eplerenone due to the increased risk of hyperkalemia.

  • Caution is also advised in patients with proteinuria and in those also using ACE inhibitors, ARBs, NSAIDs, or moderate CYP3A inhibitors.

Special Dietary and Lifestyle Recommendations

  • Monitor your potassium levels regularly, as Eplerenone can cause hyperkalemia. Avoid foods high in potassium, such as bananas, spinach, and broccoli.

  • Maintain a low-sodium diet and drink plenty of fluids to support kidney function.

  • Regular monitoring of blood pressure is recommended as the medication is used to manage hypertension.

  • Regular monitoring of the kidney function and serum creatinine levels is advised as Eplerenone can affect kidney function.
